Were you given any helpful survivorship resources by your treatment center?

Were you given any survivorship resources by your cancer treatment center that turned out to really be helpful? If yes, what was helpful? If no, what do you wish you had?

Not much that I recall - I remember talk therapy being offered but it was a swing and a miss. I wish that there had been more open discussion and options around my mental and emotional health aside from talk therapy. I felt like once I was physically cleared, much of my care team dropped out of the picture which was a very difficult transition.
